Is Digital Medicine right for me?

As part of Digital Medicine, we work with you to customize your experience so it's the most effective for you. Digital Medicine is one program that helps you manage multiple chronic conditions as applicable to you.

To join Digital Medicine, you need to be at least 18 years old and own a smartphone. There are some clinical conditions that make you not a good fit for the program. Sign up to see if you are eligible.

Am I eligible to join?

To join Digital Medicine, you need to be at least 18 years old, own a compatible smartphone or tablet and have a diagnosis of high blood pressure and/or Type 2 diabetes.*

For weight management care, you must have a body mass index (BMI) greater than 25.* Member interested in weight-loss medication must have a BMI of 30 or greater or a BMI of 27 with at least one weight-related medical condition, or already be on GLP-1 medication for weight loss.* A virtual visit with a Digital Medicine provider will be required to confirm eligibility.

In addition, you must be an active member under a Digital Medicine covered employer benefit or health insurance plan.

What will it cost?

Ochsner Digital Medicine is a covered service under many employer benefits and health insurance plans (which may be subject to applicable patient responsibility amounts).

Does this replace my regular primary care provider?

Ochsner Digital Medicine supports you in-between doctor visits to help you manage your condition as effectively as possible. The Digital Medicine clinical team monitors your readings to ensure you're on the best medication, and health coaching will provide support on building healthy lifestyle habits.

You will still need to visit your regular provider for all other care.

Is someone monitoring me 24/7?

While we are not a 24/7 monitoring service, your Digital Medicine care team will regularly review your readings and reach out to you if you are starting to trend off course. They will call to help you manage your blood pressure, Type 2 diabetes and/or weight over the phone. Your clinician can adjust your medications, and health coaching will help you build healthy habits.

What happens after I begin the enrollment process?

We’ll ship your device to your home and send you the tracking number via MyOchsner. Once you download the Digital Medicine app and submit your first reading, you're enrolled in the program!

I’m not very “tech-savvy,” is the program still right for me?

Absolutely! Your smartphone makes it easy to connect your digital device and submit readings to your care team. If you have any issues, our dedicated support team will help you every step of the way. You’ll also have access to live chat support, technical support appointments, how-to videos and setup guides.

How can I get my device?

Your device ships directly to your home, or you can pick up your device at one of our O Bar locations. View the list of O Bar locations here.

How often should I take a reading or submit my weight?

After you first join the program, it’s best to taking readings as often as possible so we can build a baseline to customize your care. Once you’ve established your baseline, we asks that you take one blood pressure reading per week if receiving high blood pressure care. If you are receiving Type 2 diabetes care, you and your clinician will work together to determine a blood sugar testing schedule that works for you. If enrolled in weight management care, we recommend sending your weight on a weekly basis.

What is the care team and what will they do?

Each patient in Digital Medicine is matched with a dedicated clinician and team of health coaches. Health coaching helps you build healthy lifestyle habits, and your clinician monitors your readings and adjusts medications as needed.

What kind of credentials does my care team have?

Your care team consists of a clinician and health coaches. Our clinicians are certified Physician Assistants (PA) or Clinical Pharmacists (Pharm-D). Health coaches are highly trained in various allied health fields including nutrition, public health, social work, health education or kinesiology. They also have specialized skills to help you make healthy changes.

How will I communicate with my care team?

Your care team will monitor all the readings you submit as part of the program and will communicate with you via phone and through MyOchsner messages. You'll also enjoy educational resources in the Digital Medicine app.

How do I send my readings to the care team?

We’ll send you a blood pressure cuff and/or glucometer that you’ll use with your smartphone to submit readings to your care team through the Digital Medicine app.
